certificate of completion: certificate form

once you have completed all of your monthly worksheets and have passed the apprentice level, master class, and working like a pro quizzes at 100% there are a few steps you need to take in order to receive your signed certificate of completion:

1. notify your learning leader that you have completed all three levels.
2. there may be some additional requirements you need to complete with your learning leader, such as correctly identifying eye shapes, and/or performing some different application techniques or looks on clients.
3. when you have completed any additional requirements, and your learning leader has signed off on all your work, log into your academy account WITH your learning leader and fill out the form below (you will not be able to see it until you have passed all three levels).

some very important things you need to know:

• you will need to log out and back in to view the form.
• do NOT submit this form without your learning leader – each learning leader will receive a list of who has filled out the form. that list will be double checked before certificates are sent out.
• you may not be able to see the form for up to 30 minutes after you completed your last quiz as our system updates your status.
• your certificate will be sent to your school
• make sure you (the student) use YOUR current future professionals makeup academy username (should be an email address) in order to make sure your certificate is not delayed in getting to you! please do NOT input a different email address than the one you use to login to the academy or you will not receive your certificate!
